Download our claim form.
It’s a PDF file, so you’ll need Adobe Acrobat Reader to open it.
2. Fill out the one page form. Make sure both you and your veterinarian sign at the bottom.
3. Fax, mail, or email the form to us. The contact information is right at the top of the form.
